A tradition in Bath County since 1913, our home care service sends
skilled nurses, therapy professionals, and home health aides
into the patient’s home. The patient’s doctor directs the
care plan. Services include skilled nursing, physical therapy and other
therapy
services, patient and family education and a home safety assessment.
Home-based care allows patients to receive quality clinical
care in the comfort of their own homes without the expense of extended
hospital
stays.
Home-bound patients who live within our service area are eligible
for Medicare payment, and most insurance companies will cover home-based
care. If you or a family member are being discharged from a hospital
and are concerned about continued needs for medical care, please ask
your doctor or discharge planner for a referral to Community House
Home Health.
Extended Care Services In Your Home
Home care staff are available on call 24 hours a day, 7 days a week.
Recovering from illness and injury often continues after a hospital
stay. Our transitional and home care services make it possible to remain
at home or in your home community, close to your family and friends,
and continue in the care of doctors and nurses who know you and understand
your needs.

Life Alarm System
When a resident of Bath or Highland Counties is home-bound and lives alone full-time or part-time, Bath Community Hospital will pay for the initial installation charge of a Life Alarm System. A Life Alarm System consists of a pendant worn around the person’s neck and an alarm system placed by the phone. When a medical emergency occurs, such as a fall or sudden illness, the person presses the button on the pendant activating a two-way voice alarm system that calls a monitoring center. The call is acknowledged within 60 seconds and neighbors and the local 911 number are called, if needed.
There is a monthly fee of $30.00 which is paid for by the family members of the client. Financial Assistance is available based on need. For more information, call 540-839-7017.
Hospice Care
Patients and their families need special care and support when facing
end-of-life issues. Hospice care focuses on providing comfort and dignity
in the comfort of home and provides special services such as pain management,
family education, and bereavement support, which are helpful to the
entire family. Care plans are individualized and directed by the patient’s
personal physician.
